Research has shown that Stroke affects 500,000 individuals a year and is the leading cause of disability in older adults. Stroke occurs when there is a sudden disruption of the blood flow to the brain either by blood vessel blockage or a burst artery. Brain cells that are deprived of blood either become damaged or die. Those that die do not regain function and cause disability. The area of the brain that has been affected determines what problems a person has.
The environment for the stroke Programme at Cross-Point is focused on safety, comfort and recovery. Patient rooms and the large therapy gym are located on the same unit, allowing easy communication between patients and rehabilitation experts.
Cross-Point’s Stroke Centre is led by a Stroke Programme Physician, who is a MDC certified physiatrist (specialized rehabilitation physician). This physiatrist leads a team of stroke rehabilitation experts including Neurologists, Nurses, Physiotherapists, Occupational therapists, Speech therapists, Psychologists and registered Dietitians. They are supported by a team of competent administrative staff.
The stroke rehabilitation programme is customized to fit each patient and family. The stroke rehabilitation team works with the patient and the family to identify their goals, complete a comprehensive assessment, and then maps out a patient-specific plan.
The Cross-Point Stroke programme utilises evidence based intervention; that is, the treatment provided at Cross-Point includes those things that clinical research have proven to be best for advancing return of health and function after stroke. The entire team, nursing as well as therapy, is trained to support and to facilitate return of function through activity based intervention.
The benefits of rehabilitation do not stop after a patient has successfully demonstrated improved function and has returned home. Stroke recovery can continue for months and years, and the Cross-Point programme is designed to support that continued recovery. Cross-Point’s Follow-Up System of Care provides individuals and families with continued access to their rehabilitation physician and a team of expert rehabilitation professionals. This team provides a variety of services to help patients continue to regain or to maintain function, including case management for lifetime rehabilitation care.
